Specimen Data Forms

Every sample submitted to the Plant Clinic following the How to submit a sample guide should be accompanied by one of the following forms. All three forms are in .pdf format which is viewable in Acrobat Reader. If you do not have Acrobat Reader you can download it free of charge from the Acrobat website. Once you have Acrobat Reader, download the appropriate form for your sample, print it out, and after filling it out completely include it with your sample and appropriate Fee.

• Form to use with field crops.

• Nematode Soil Sample Form: For this form print out page 5 of the Report on Plant Disease No. 1100 and be sure to follow the detailed instructions in the rest of the report.

• Form to use with all other samples.

University of Illinois Plant Clinic is open May 1 through September 15th. Samples will not be received at other times.

Note: Diagnoses and recommended controls by the University of Illinois Plant Clinic are based solely on the material and information submitted. The less representative the sample, and the less complete the information provided, the greater the chance for misdiagnosis.
